Promoting Safe Transitions to College Mental Health Services: Lessons Learned

The transition from high school to college is a landmark event for young people and their families, but for youth with mental health issues, the transition presents extra challenges. Proactive, developmentally-based, transition planning can play an important role in the prevention and early intervention of mental health issues in college-bound patients. Presenters address potential barriers to achieving a successful transition and the necessary elements of a transition plan, e.g., adequate time spent planning and preparing, matching of resource(s) available with clinical needs, doing a realistic assessment of college readiness, providing adolescents with a proper education in or the opportunity to practice managing their own mental health, and working through the changing physician/patient/parent dynamics.
