The frequency of use of atypical antipsychotics in children has continued to rise, making it highly important that child and adolescent psychiatrists are aware that starting atypical antipsychotics in children can be much easier than stopping them. The use of these medications must be considered carefully and the difficulties involved in discontinuing use fully understood. Attendees review what is known of the neurobiological action of the atypical antipsychotic in children, the complications they can cause, and withdrawal syndromes including agitation, irritability, hyperactivity, and aggression. Since the literature offers almost no practical help on how to taper and safely discontinue use, case presentations offer the best source of information, illustrating successful, safe tapering and discontinuation in complex situations.
